To help confirm if you need cover for any pre-existing medical conditions, we have a few
simple questions to establish what cover you need.

Please select the account type that you have with the Co-operative Bank from the drop down below and enter your account number to proceed.

If you are currently in the process of applying for an Everyday Extra account with the Co-operative Bank, please select Everyday Extra and continue without a Bank Account Number.

Please Note: The medical endorsement is only valid if you have a current valid account with The Co-operative Bank.

If you do not have your account number to hand, you can add this to your medical assessment record at a later date by connecting with one of our live chat agents. This is done by clicking the live chat button on the right of your screen.

During the last two years, has anyone covered under this policy had any medical or psychological condition:

  • which has resulted in symptoms, or;
  • for which any form of treatment or prescribed medication has been required, or;
  • for which they have required any medical consultation, investigation or follow-up/check-up?

Has anyone covered under this policy EVER had any cardiovascular or circulatory condition?

For example:

  • a heart attack, angina, chest pain(s) or any other heart condition;
  • high blood pressure, blood clots, raised cholesterol, aneurysm or any circulatory disease;
  • any form of stroke, transient ischaemic attack or brain haemorrhage.

Some conditions are covered automatically and free of charge, provided:

  1. the insured person is not awaiting surgery for the condition; and
  2. the insured person has been fully discharged from any post-operative follow-up.
